I checked your form (https://www.jotform.com/71286478612665) on the site you have provided (http://www.charlasartrosis.com/registro_bogota/ ) and cloned it on my end. The form seems working fine on my end, I was able to submit the form and register without an issue.
Are you still experiencing the issue on your end? If the issue still persists, can you please ask them to clear their browser's cache and try submitting your form again?
