Why my logo on top of the form does not display at all on iPhone?

I assume you have mentioned about the following form: http://www.jotform.me/form/71290346658462 

I have checked it on iPhone and I have seen that the logo doesn't appear.

I have checked the form in the form builder and there is no logo as you can see the screenshot below:

Also, I have cloned your form on my side and there was not any logo on my side inside the builder. When I have previewed the form, your logo has appeared on the preview&publish mode.

I have uploaded another logo and it was appearing correctly in the builder.

Could you please try re-uploading the logo again? If the issue will not be solved, I am going to forward it to our software development team in order to investigate the issue with the logo.

Moreover, I have cleared your form caches in order to avoid any possible issue.

Also, could you please add a mobile responsive widget to your form in order to make it mobile friendlier?
