Are your forms legit?

today i recived this form, is this legit. Reward about giveaway.

When I checked form and user having this form, the only field violates our terms of use it SSN field. We do not allow our user to collect personal sensitive information such as social  security number or credit card details. 

I'm not sure if this form is using for a real phishing attempt. However, I'm going to suspend this user.

I asked for it because i am inside a giveaway on youtube channel, and there was a link for a winner . I think youtuber tech savage is from california and this is .ca form.

And i have to check it is legit.
