Some forms not showing up on my website

    There are several forms on my website not showing up on the page. I have checked that the code is still embedded and it all looks fine. I have put the link to one of the affected pages.

    Please let me know what the problem may be.

    I have checked the web page that you shared with us. However, I could not encounter any issue on my side. As you can see below screenshot:

    I have checked your source code and could see that you have injected with the "Embed" method.

    Could you please remove this script and re-embed your form with the iFrame method? Here is a guide helps to you: https://www.jotform.com/help/148-Getting-the-form-iFrame-code

    If the issue still persists, you can change the form link inside of your embed code, from

    src="http://form.jotformpro.com/jsform/43082161160948"

    to

    src="http://form.jotform.com/jsform/43082161160948"
