My form is disabled

Gostaria de saber qual o campo em que posso estar violando os termos de uso do jotform. Pois um formulário que estou criando está como desativado.

I have checked your account and your form. Your form contains "Password" field. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

I have deleted this field and re-activated your account. Your form is enabled now, you can log in to your account.
