I checked your form and I can see that the Disable on a Date feature is enabled. The form expiration date was on April 10, 2017. To enable your form again, you need to change the the Form Status to Enable or set another expiration date. See image below:
1. Login to your account and open your form.
2. Go to the Settings page section and select the Form Settings tab.
3. Change the Form Status to Enabled or update the Expire Date to your own preferences.
