I tried the suggested fix, still doesn't work. Tried with Chrome, Chrome incognito and Safari.
Even with the google API key, I can't get the autocomplete feature to work. I tried taking the key away, refreshing and adding it back, but it still doesn't work.
I have added some conditional logic to the page. I moved it after the autocomplete box thinking that might help, but it did not. Here is the link to the form. This would really help submissions if we can get it to work!

I have checked your form and I can see the widget does not work; however, I have cloned it and added my API key, I can see it works fine:
I would suggest you to make sure your key does not have any restriction, this can be checked on the Google API manager and clicking on the edit button of your API key:
Make sure the restriction is set to none:
