I have a wordpress website. I want to have a registration/ medical history form on it that patients can fill in

    I want patients to be able to fill in the form on the website which is a medical history template, save it and have the information sent to me as well as auto-upload to Google Drive (where I keep my patient files).  Is this possible?  I dont want patients to have to leave the website.  Right now, patients have to download the form to their computer, fill in and save, then email me separately.  This is impractical.  Is there a solution?

    You can create an online form in JotForm so that your patients will no longer have to download the form on your site and so that you don't have to do the process you have mentioned.

    I would suggest you please create an account in JotForm if you have not signed up yet. Please sign up here: https://www.jotform.com/signup.

    You can integrate your form with Google Drive where all the submission and submissions data will be saved on your Google Drive account. Here's the guide on How to Integrate a Form with Google Drive.

    After the users submit your form, you can also receive email alerts of their submission. You can set up an Email Notification on your form. Please refer to this guide: Setting Up Email Notifications.
