I requested and recieved a credit yesterday for invoice # 20171301006. However, my service has now been suspended. I need the service reinstated immediately since we have paid for our May subscription on invoice # 20171280844. Please reinstate our subscription immediately.

It was done as requested, that is the reason why your account is on free status. Your account is suspended, because you have reached the upload space limit. You can simply delete submissions that contain uploaded files to free up space, or upgrade to a paid plan if you want to: https://www.jotform.com/help/331-How-to-Upgrade-My-Account
If you want to upgrade through check, or purchase order, please note that your account will be upgraded until the payment is received, and that can take up to a month, so my recommendation is to upgrade through a credit card if you want it to be immediately.
