Page next button is not working on my form

and also, sometimes, when I go to Preview Form and fill out the first page and click the next button, nothing happens.

I have cloned your JotForm to my account to test. As I check the form I see that not only the page next button is not working but also the form collapse is also not working. 

I am currently running some test on the cloned version of the form. I'll get back to you here with my findings soon.

I have checked your JotForm and I have tried deleting all the conditions on the cloned form to see if they were causing the issue. But the issue still persists after deleting the conditions. It looks like some fields are causing the issue. I notice that the calculations are also not working on the form.

Could you try reverting the form to an earlier working version from the form revision history? Please refer to the guide below that can help you with reverting the form to an earlier version.
