Can I incorporate a pdf form that needs to be filled out and signed and embed into my site?

I have a pdf form that I need to incorporate on my site, but I need it filled out and signed before its submitted. Is there anything I can do here to achieve my goal?

Also, the forms I have created already, can they be converted to pdf files and be filled out and signed as pdf files?
