Please check this form https://form.jotformpro.com/71306517715959 to have the underline displayed in the Preview Mode.
The underline is displayed in the Build Mode, but does not displayed in the Preview Mode.

Please fix the Preview Mode for the underline to be displayed.

I have checked your form and I can see what you're reporting; however, I have removed and added the underlined setting from your text and it worked now, do note that this should have been set through the text editor tools, highlighting the text, clicking on the underline option and saving changes:

Thank for the responding, now I know your Jotform platform does not work with the tags <u>underline</u> for underline, instead it must have been set through the text editor tools.

Hello,
Yes, the HTML tags that can be used are now limited on the form builder.
The <u> tag is now replaced by <span> and with this tag the style attribute is how the text gets formatted on the form builder. Example: <span style="text-decoration:underline;"> is how the form builder sets an underline on texts.
This setting has been put in place as a security measure to limit the probability of using HTML tags to do XSS injection on forms.
