I'm sorry for the trouble caused to you.
I did test your form and I am able to replicate the issue you are having. I am not sure what is causing this issue but "PDF Embedder" widget wizard does not load in your form. I tested the "PDF Embedder" widget in one of my own form and the widget wizard loads just fine.
I am reporting this issue to our backend team. We will get back to you as soon as we have any update form them. For now, I would suggest you to please delete the "PDF Embedder" widget and add it again.

Hi, we apologize for any inconvenience. The problem has been fixed. Btw for your information, we made an update to it last month that you will have to upload the PDF file from now on instead of using a direct url. Old integration still supported so you don't have to reintegrate your widget. Please test it out and let us know if how it turns out for you.

Hi, I'm sorry about that. I've checked your form and I found out a problem on our side. We will fix it. For the time being, please try to rename your file and avoid using the ampersand symbol or the &. Just rename it and try it again if you will be able to preview the uploaded PDF file. Let me know how that turns out.
