i have created a form and it works without any problem.now the problem is when i integrate the form with inshightly CRM the submit page is not displaying...when i click submit it stuck with empty page....didnot redirect to the thank you page....And i cant get the email notification too when user fill the form...

I cloned your form (https://www.jotform.com/70821331982455) and integrate it with Insightly CRM on my end. I was able to be redirected to the Thank You page upon submitting the cloned form.
Could you please check if you have integrated your form properly with Insightly CRM? Here's the guide on How to Integrate your form with Insightly CRM.
I also noticed that there is no FullName Field on your form, please add this and match it to the fields on Insightly so that it will work.

Would you please try to re-integrate your Insightly CRM application by following the user guide which I have shared with you above and please also consider what my colleague mentioned above.
If you encounter any issue, please give us further details in which step did you hang out.
