Although the form you showed on the link you provided is theoretically possible to do with someone skilled with CSS and HTML, I'm afraid you still have to build your own form from scratch, then have your designer make the visual changes similar to the form you gave as an example.

It's worth mentioning that we have plenty of poll templates to choose from so I guess that would be a good way to start.
