I did check your form "Emergency Reference Information Form" and found that you have not configured it to redirect to any custom / external thank you page. Please check the screenshot below:

I also tested your form by sending a test submission and it did not display any blank thank you page. It shows default thank you message which is configured in your form. Please check the screenshot below:
