it is hosted by goDaddy and they tell me I just need to give them a link to imbed so customers will be taken to where they can fill out the docs online and send them back to me.

Custom form building services is not something we offer at this time.  If you would like to try creating them on your own, we will be happy to help with any questions you have along the way. Most form elements can be adding to form simply by dragging and dropping them into the form.
