I am having the same issue, When i select "Lead" there should be fields that correspond with my fields on SalesForce. Those fields are not showing. I am also showing a disconnect of integration as of 1:30am(PST) new leads have not come in.
Any information if this is a Jotform issue or a Saleforce issue?

I have checked the Salesforce integration at my end and it seems to be working fine at my end. Could you try removing the Salesforce integration on the form and reintegrate it to see if that works?

Thank you for your attention. On further inspection my issue seems to be linked to a 500 error when Jotform communicates with Salesforce. It might be a credentials issue (expired password). I am working on getting new credentials. I will update once things have been tested.
