Adding matrix style field with radio buttons

Hi,

I am trying to produce a matrix style scale rating question in your job form, and wondering the best way to do that. Right now I'm using the matrixselect widget, however this allows users to select multiple items per line item and I need to restrict choice to one item per line. Please see a screenshot below of what I'm looking to produce. You can see the matrix I choose, by using the link of the jotform below. 

The Input Table (Matrix) field is a good choice. When the Input Type is set to the Radio Button it will restrict the selection to one item per row.
