Can you embed the same "form" in more than one web page?
I have the same form embeded in two pages on the same web site.
The form is set to email both a notification email and a Autoresponder email
It is also set up to send the data as a line item in a "Google Spread Sheet" in the cloud
The URL for the second page is

This is the first month of operation of the Web Site
What I have observed to date is
1/ Some times I receive two entries in the "Google spread sheet " for the same booking.
2/ One person trying to send an application has informed us he is unable to submit the form as it advising them that required fields have not been completed when they have been.

Hi,
Sure, you can embed your form more than one site. However, I would like to state that this will directly affect the form display limit.
I can recommend you to try setting unique submissions for multiple google spreadsheet submissions.
How to Set Unique Submissions on a Form
I have just sent you a test submission in order to check your second problem. However, I cannot replicate the issue.
I have also cleared cache of your forms. Please let us know if your problem persists.
