Can you please give us the Form ID/URL so we can have a closer look? Your account doesn't seem to have any forms at the moment. And if the form was embedded on a page on your website, share with us the URL too so we can check it from there.
It would also be helpful if you could give more details about your issue (e.g. which box are you referring to, what's the name of the field, etc.). Thanks! 😊
