Checkboxes Not Working

Checkboxes are unable to be checked, even when viewing in incognito mode, no ad block software is running, not working in another browser (Microsoft edge, form built in chrome).

I checked the form but as coming from my observation, the checkboxes are getting selected. However, the color of the select highlight is white, and it's close to the color of the background, which why it somewhat gives an impression of not being selected.

By changing the color, it should resolve the issue.
