I need to create a multi-payment form

Hi,

I Tried to create the form for my website.. Invested more than 5-7 days t but at last only i came to know that i could not add additionaly payment method on some forms. If you check on the url provided below. Ther are some fields that requires payment when its clicked "Yes". This has been the major issue for me now. Please Suggest
