My form is disabled due to requesting usernames and passwords

    Can not make my form to work. It says that it is diabled but it i have check the settings and it was enabled https://form.jotformeu.com/71291992860364

    I checked your account and it was suspended by our phishing detector for requesting usernames and passwords:

    We do not allow for the collection of usernames or passwords through forms for any reason. 

    I removed those fields from the form and activated your account.  Your forms should be usable once again.
