Extra fields in published form that are NOT on Builder

    I have emailed a couple of different times and also posted to the forum and still have not gotten a matter taken care of.

    There are items on a form for Youth Camp registration in our published form that are not in the built form, so I cannot delete them. I have posted a note for registrants, but it looks incredibly unprofessional.

    Here's a picture of the extra fields as showing up on the published form at the end of "Camper 1" information.

    As requested before by someone at JotForm, I have cleared all cache, both it JotForm and in various browsers, and these fields are still showing up.

    Sorry for the inconvenience, 

    I checked your form and I can see that you have a lot of conditions that are missing fields, please see:

    I cloned your form, deleted these conditions and now the form displays normally. Please see below:

    Please delete these conditions, that will fix the issue.
