Upon checking your account, it showed that our phishing filter has detected one of your forms to be suspicious. This filter acts similar to email anti-spam filters which scans and suspends forms without any human intervention.
But upon review, it looks like to be a false positive. This may have been triggered by the term 'credit' on your form. With this, I have reactivated your account and whitelisted your form. Your account should be fine now and your forms should be viewable.
