My form domain will not work

when my applicants prince the link jotform and i created, it gives them a 404 message

When I checked your account I saw that it is autosuspended by our phishing system. Your account was suspended because of imported Sign In form. Login forms are not allowed. Considering that you have deleted mentioned form and form is located inside of Trash folder, I have purge it from your account and I have activated your account.  

Latest form that you have edited http://www.jotform.us/form/71356163684158, does not have custom URL. If this form should be shown when accessing to the link you provided, I would suggest you to recreate custom URL of that form. 

How to Create a Custom URL of a Form?
