We have some forms that report lead sources back to our salesforce, while other forms do not.
How can we be sure that all forms are set up to correctly send system information to our salesforce. Below is the form that we currently do not receive utm info for.

I have checked your form and it seems properly integrated, I have also cloned your form and I can see the submissions are sending to SalesForce.
I have set up the integration like this:
Did your submissions stop sending to your leads list?
Or are only some submissions that were not sent to the list?
Could you please also try removing the integration and adding it once again?
If the issue still persists, please let us know if this happens with all the fields or if there are only some fields that are not sending.
Also, which field are you using to capture the UTM source?
