How long the form will be available?

I created and published a form. Now, when I open the link of the form, it says that "The form is currently unavailable". What is the problem?

I checked your form and I found that you have set your form to disable on May 17th, hence why it's showing the unavailable message, please see:

If you want to enable your form again, please remove the "Disable on date" rule from the form settings.
