Why is my form disabled and how can I re-enable it?

I didn't disable this form, and when I go into jotform, it appears as if everything is working.

I have checked your account and found that it got suspended because your form is collecting banking information, without inserting a signature field, which violates our terms of use.

Please see our terms of use, especially sections "Phishing" and "Collecting Sensitive Information" here: https://www.jotform.com/help/8-Terms-of-Use

If you accept our terms of use, and declare that you will add a signature field as soon as you get back access, we will consider re-activating your account.

If you need further assistance please let us know. We will be happy to help. Thank you.

I accept those terms of use (didn't realize, sorry!), and we will happily add a signature field as soon as I get back access.

    Thanks,

    Grove

Thanks! In addition to the e-signature, I'm trying to enable the encrypted forms feature, following these steps: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

However, I do not see any option in my settings on this form to turn on encryption. Do you know why this might be? Is there a way for me to use this feature with this form?

    Thanks!

    Grove
