Unable to embed form in WordPress.

I am setting up on wordpress.com I embedded your form fine and now I cannot please help

I am not sure if I have understood your question correctly. Do you mean to say that you are unable to embed your form in WordPress? 

I did check the website URL which you have shared and it seems to display the embedded form without any problem. I was also able to submit your form successfully. Please check the screenshot below:
