Need instructions- Create registration form and see if it linked with our website

Hi,

I heard good things on your site and we want to test and see if we can create a patient registration online form and linked directly to our website and then notify us through an email when it's complete. Can you help with instruction?

Also, I see that you have the free for 100/mo. submission, etc.  can you explain on that plan and bronze for me?

Creating forms is quite easy using JotForm.  Here is our basic guide on how to get started:

Email notifications are set up automatically for forms but you can include additional notifications if you'd like:

Once your form is created, adding the form to your web page is also a simply process:

As far as subscriptions go, Free accounts allow up to 100 regular submissions or 10 payment submissions per calendar month.  There are also storage limits as described here:

A bronze subscription would allow for up to 1,000 submissions of any type per calendar month with drastically increased storage limits.  A bronze subscription would also allow for removing JotForm branding from forms.
