Unfortunately, I wasn't able to replicate the issue. I opened your form in Google Chrome and in Mozilla Firefox. The background image and image logo is showing properly.
The color of your logo is white and the background is transparent. The logo will not be visible if the background image is not working. The problem is that the background is taking time to load when I checked it. It takes me 5 seconds to fully load the background image.

It is possible that some of your users have slow internet connection. Are you experiencing this problem on your end as well?
I suggest you use a smaller background pattern image. You may also use a plain background color if you like.
