    Is ther a way to take the answers to questions on the form and populate other programs, such as an EMR for a medical practice?

    I think you could post the data to your own EMR system database and then pull the records of the patient based on an ID  (unique key)

    Your form should include a field to catch such ID and the post data. Of course this workaround would envolve a certain level of PHP programming . Please check this guide where data is posted to a MySql database using PHP programming
