An individual received an error message when submitting a form

The above is the message that the individual received when trying to submit the form. We've had successful submissions so I'm not sure what is wrong! 

Unfortunately, we have experienced a technical glitch with our server and it has been quickly resolved by our backend team. The forms should be working fine by now. Please check and let us know if you experience any further issues. We will be happy to take a look again.
