Why is my form disabled?

    I'm getting this message when I try to view my live form. Have I violated something to have caused this?

    Your account was suspended because of phishing fields. I checked your form and I can see that you're asking for facebook name, username, and password which violates our terms of use

    If you want we can remove the guilty fields so we can re-activate your account. We'll wait for your response. Thank you.

    I will be setting up new web-based accounts and need to have the user specify these fields. This information will not be used to get into any existing accounts. The new accounts will not be created on facebook, pinterest, twitter, snapchat, linkedin, or any other existing social media website. The password field will be manually moderated to ensure format and length integrity are maintained. The request for the facebook username is so I can include that user into an existing facebook user group.

    I hope this is sufficient to reactivate my account.

    As to collecting passwords, please note that we do not have exceptions about any use cases related to them. It is simply against our terms of use to collect passwords, no matter what the reason is.
