My survey is being cut off when I embed into my website

Please help -- I can't figure out why the bottom of my survey is being cut of when I embed it. Please view here:

The jotform is on the right. The bottom "subscribe" button does not appear.

I see that the form is embedded using Javascript default embed method that is adjusting the height automatically. Please try embedding the form using the iframe embed code by using the colored section only as mentioned in the screenshot below:

You may also change the height of the iframe to display the form correctly on the web page.
