Do you have a charity donation form that accommodates recurring billing for Stripe?

I need the option to allow donors to subscribe for monthly donations as well as one-time gifts and specify the following: frequency (weekly or monthly), Day of the month the donation will be charged. Also, a check box to allow the donor to choose ACH or credit card payment, along with text boxes for the appropriate information (routing and account number, CC's etc.) I

Unfortunately, you can't define periodic subscriptions and one-time donations on the same form. You can collect subscriptions or get one-time donations.

You can create two forms, and add links to them on a different form to have both options available for your users. You can also add these two form into another form using iFrame embed widget to have them available on the same page.
