The booking form is not working on an iPhone. Can you please advise how I can rectofy this ASAP. THanks in advance.

Could you please tell us more about the problem, since when I checked the form on iPhone 6s it was showing fine:
Are you unable to enter the values, submit the form or is it something else?
Also, are you viewing the form directly or your form is embedded in the website?
