Can we name our forms differently that the 20 numbers it automatically generates?

We would like the forms to be named by date, service log ID, or something much shorter.

Is there a way to make this happen?  Please advise....

I am assuming you are asking for the change of the number at the end of form url. You can customize your form url by following this guide:
