Form is Disabled

    Ive had to ask this question 3 times now with no response. 

     

    My form is disabled and no matter what I do I cannot get it back on.

    Opening multiple threads will not help you to speed up an answer, all questions are answered in the same order they entered, so the more questions you post, the later you'll get a response from us. This particular concern was answered in another question of you, if you have further concerns, please follow up in that single thread.
