How do I get my logo to print out on a submission?

    I'm using the form to fill out myself during a consultation with a client. Then I email them a pdf printout. Would like my logo and info to show on the print out of the form and data, but can't seem to figure that out. I added the logo at top, then added another as an image in the form but neither print out.

    Additionally, is it possible to input online data but have it not show up on printout?

    I think you have just not enabled the Show Headers and Text option in the Submissions page of the form.

    Additionally, is it possible to input online data but have it not show up on printout?

    On the same option screen in the submission page of the form, you can Uncheck the fields that you do not want to be included in the print out.
