How can I integration Docusign sucessfully

Hello I am trying to have the form so it can be signed by each manager then sent to the next manager when they enter the email and name through the docusign integration. But when I enter a name and email it just says fix errors but dont know what errors it is referencing to?

I have tested your form and this error message was shown.

Please try to reintegrate your form with Docusign. You can remove existing widget and add new one.
