I see that you have included "searchenginesmd" in your name for this post so I assumed that is your account username.

I checked the status of your account and I see that it is in the autosuspension status. This is the reason why your account says it is not available. When I checked what triggered the suspension I found out that there is a form (http://www.jotform.us/form/71272074584155) in your account guilty of violating our terms.

Please note that collecting of sensitive information such as login passwords in our forms is prohibited.

We have an anti-phishing tool that scans every form and marks any form suspected of phishing activity then suspends the account that owns it after.

I have removed the field shown in the image above from the said form and reactivated your account.

If you have any further questions regarding this, please feel free to contact us again anytime.

    Thank you.

Understood and I had started a new form that did not include that
    information.

Also the "password" was used as an example and a suggestion as to what the
    client might use.

Thank you for the clarification. We understand your concern, however, collecting login credentials particularly username and password is not allowed in our forms. Even the use of the word "password", in any means possible, would alert our anti-phishing tool and trigger the suspension.
