Why am I getting an out of date form, in Dutch?

    I have now received two forms with the form details in Dutch, while the content submitted is in English.  

    I attach screenshots, one showing the form as it appears on the Jotform website, the other as I received it by email.

    I cannot find this exact form on my website.  I revised the form recently, so I have no idea how people are accessing this version of the form.

    My website is http://www.being-in-voice.com  I have checked every link I can find!

    I'd appreciate your early advice.

    Hello flloyd2010.  I cannot tell why you have a Dutch language in your email notification but what I can tell is that the form is in your account and is entitled "Contact" with the form ID 21223732681852.  If you cannot find it, you can simply search for the form using the search field on top of the list in your My Forms page:
