Why my form won't rename?

Hi I have been trhough the nline help tools and tried everythign i can find however my form won't rename.

It comes up with the name Music With Friends Tickets, but by the form settings should be named Teacher Performance Submissions

Please help.

1) Go to Settings-Form Settings-Show more:

2) Scroll a bit down and you can change the form title:
