My forms keep saying they are disabled?

Asked by gazaleyewear on May 26, 2017 at 10:25 AM

The top two forms both say disabled but I have checked settings several times.  Enabled and disabled and enabled them several times.  Please help asap, I have staff that need to use this feature asap, thanks!

Upon inspection of your account I found that it got suspended because your form "Direct Deposit Form" is requesting banking information without adding a signature field, and that violates our terms of use.

Please see our terms of use, especially sections "Phishing" and "Collecting Sensitive Information" here: https://www.jotform.com/help/8-Terms-of-Use

If you accept our terms of use, and declare that you will add a signature field as soon as you have access to your account, we may consider re-activating your account.

If you need further assistance please let us know. We will be happy to help. Thank you.
