My forms keep saying they are disabled?

    The top two forms both say disabled but I have checked settings several times.  Enabled and disabled and enabled them several times.  Please help asap, I have staff that need to use this feature asap, thanks!

    Upon inspection of your account I found that it got suspended because your form "Direct Deposit Form" is requesting banking information without adding a signature field, and that violates our terms of use.

    Okay I am adding it now thanks.  It was hard to find the answer to my questions btw, i did not see any notifications or emails when i logged back in today.  

     

    Update:  I added a signature plus a captcha.  Thanks for the help!
