Why does it say form not found when I try to preview the form that I have created?

Hi, 

Your account has been auto-suspended due to suspected phishing activity. Your form "Credit/Debit Card Authorization Form" is collecting sensitive credit card information which is against our terms of use. 

In order to get your account re-activated, this form will have to be deleted, as we don't allow any way of collecting credit card information.
