The following form and all others show disabled. I'm not sure what I'm doing that is causing the problem. Last time I emailed, my Credit Card Authorization form was just deleted with no response to me. I just created another one entitled "Payment Authorization Form" and it happened again! I am not directly processing payments nor am I collecting card info. I just need signed authorization for an amount. My submissions are minimal and not even close to maximum levels. Please let me know how to remedy this situation (without deleting this form again, if possible please) as I am obviously doing something consistently that I shouldn't be;)Page URL:

The card texts in the form triggered our system to auto suspend your account. I see that you are asking users for the last 4-digit number which is fine. Just make sure to keep it that way and don't ask for the full numbers as well as the CVV code. I have reactivated your account, so please check again.

Thank you, I did request the CVV code so I'll make sure it is removed from all forms:)
