Form Not Found. Form Disabled....

I made a form for people to fill out for staff positions, and doesn't violate any rules that I have seen. Usually it's just a glitch from what I have seen and would like it unlocked... Thanks!

I have checked your account and found that it was automatically suspended by the phishing filter. This filter acts similar to email anti-spam filters which scans and suspends forms without any human intervention. 

Upon reviewing your form however, I did not find anything suspicious with any of your fields.As a result, I went ahead and reactivated your account. You should be able to access your forms now.
